Synapticure was selected by Medicare (CMS) as an Established Program for the GUIDE model to support families living with dementia. This enables us to offer GUIDE services in all 50 states. We will begin supporting GUIDE beneficiaries in July 2024.

What is GUIDE?

The Guiding an Improved Dementia Experience (GUIDE) program offers comprehensive care coordination for patients and caregivers living with Dementia, as well as access to in-home or facility-based respite care. All of this is available at no cost to beneficiaries.

What is provided?

Expert clinical care to support diagnosis and treatment of physical, cognitive, and behavioral symptoms of Dementia
Dedicated care navigation available 24/7 to help with anything patients and caregivers need
Respite care services to give caregivers relief, and provide patients with skilled companionship and support (up to $2,500 / year, or ~80 hours of care)

Eligibility requirements

Beneficiaries with Traditional Medicare (not Medicare Advantage)
A confirmed dementia diagnosis (including Alzheimer's, LBD, FTD, vascular dementia, etc.)
Living at home or in the community (not in long-term care)
Not participating in Hospice or a PACE organization

What is included?

Appointments with specialized Neurologists in less than 1 week, with no referral required
Integrated behavioral health care including psychiatrists and therapists who can support patients and caregivers
24/7 access to care coordination to help with scheduling appointments, obtaining medications or medical equipment, arranging home-based services or transportation, accessing financial or legal planning and support, access to meals and other supports, and caregiver skills education and training including support groups
Coordination of respite care coverage including scheduling and logistics

This visit was like a breath of fresh air. After being diagnosed in 2017 and fortunately, my progression has been slow, we felt we were at a standstill. After speaking with Dr Geraldi, we feel there is more hope in even the smallest of things, day-to-day living and future for what might be on the horizon as far as treatment. We are extremely happy we found Synapticure.

Dr. Geraldi was very thorough and gave good advice which I will for sure follow. She seemed to be on the cutting edge on how best to fight the progression of ALS!!
