In addition to evaluating physical and mental health, we also identify potential challenges and understand social factors that could impact care. By doing this, we can tailor our support to your journey, allowing our experts to identify the most effective treatments and interventions.

Customized Care Plans

Based on the assessment, we work with you and your caregiver to develop a customized care plan that addresses both immediate and long-term needs. This plan outlines the steps we’ll take together to manage your condition and improve your quality of life. Whether it’s coordinating mental health care, navigating caregiver support, managing medications, or setting up home safety measures, every aspect of your care is planned with your needs in mind. These plans are flexible and adaptable as needs evolve.

Medication Management

Managing medications can be complex, especially for those with dementia who may be taking multiple prescriptions. Our care navigators ensure that your medications are managed safely and effectively. We coordinate with your healthcare providers so you’re receiving the right medications at the right times. Additionally, we offer guidance and education to caregivers, helping them understand the purpose of each medication and how to administer it correctly.
