Caring for someone with or living with dementia can be overwhelming, but you don’t have to do it alone. Synapticure provides wraparound care, meaning you have access to support whenever you need it. Our 24/7 phone line is always available, and our care navigators are here to help with everything from arranging transportation, navigating local services, and coordinating mental health support or in-home physical therapy, occupational therapy, or speech-language therapy.

We continue to support you throughout your journey, providing ongoing assistance to ensure you and your loved one have the resources and care you need. Through the GUIDE program, eligible Medicare beneficiaries can access these services at no extra cost.

24/7 Phone Line

Whether it’s a sudden concern about symptoms, questions about medication, or an unexpected event, you can connect with a provider or nurse at any time who can provide clinica guidance and support. Care Navigation

Navigating the healthcare system can be overwhelming, especially when dealing with a complex condition like dementia. That’s where our care navigators come in. They act as your personal guides, helping you access all the services you need, from arranging in-home services to scheduling appointments with specialists. The GUIDE program covers this care navigation service, ensuring you have the support you need to coordinate every aspect of your care. This not only eases the burden on caregivers but also ensures that patients receive comprehensive, well-coordinated care.

Ongoing Support

Our commitment to you doesn’t end with a single visit or phone call. We provide ongoing support throughout your journey. Whether you need regular check-ins, help managing daily challenges, or assistance with long-term planning, we’re here for you. Our wraparound care approach means that you always have someone in your corner, helping you navigate the ups and downs of living with dementia.
